Aim: A significant number of people with intellectual disability in England & Wales are placed in services away from their natural communities. This paper reports on the summary findings of a review undertaken on behalf of the Social Care Institute for Excellence into out of area placements. The aim was to establish the frequency, user characteristics, costs, and organizational determinants associated with such placements. Method: The review involved a systematic analysis of existing literature together with a practice survey; the latter was achieved via on-line consultation, workshops and conferences. Results: Rates of out of area placement ranged from 3-90% across commissioning areas and there was some evidence of this being an increasing trend. Challenging behaviour, mental health difficulties and autism were amongst identified risk factors. A number of systemic weaknesses in the commissioning of care that fuelled out of area placements were identified. Conclusions: Despite some important caveats, the overarching conclusion of the review was that such placements had negative consequences for the individuals and families concerned, and that their impact on both exporting and receiving areas was also detrimental. A series of recommendations for addressing the issues identified are proposed.
